IBC Forms and Tools

Preferred submission method: Single email to irb@georgiasouthern.  IBC applications can be signed with a physical signature (sign with a pen and then scan the document for submission) or signed electronically.  To sign electronically, please use the Adobe Digital ID or DocuSign.

Renewal or Modification of an Existing Protocol

To renew or modify an existing approved application

For renewals: Must include an updated Laboratory Audit and current Training Certificates for ALL personnel.

IBC Termination Form To close an IBC protocol.
